STANFORD — Tami Anne Hershman, 41, of Danville, died Sunday, April 8, 2012, at the UK Medical Center in Lexington.
Born Jan. 8, 1971, in Los Angeles County, Calif., she was the daughter of the late Dan and Faye Klugman Hershman. She was a homemaker.
Survivors include her husband, Michael Phillips of Danville, Ky.; four children, Tori Proctor (John) of Smyrna, Ga., Ryan Phillips of Junction City, Ky., Hannah Carpenter and Mia Carpenter; and a sister, Debra Hershman-Rubens (David) of Smyrna, Ga.
A graveside service will be 11 a.m. Saturday at K.P. Hall Cemetery with Chaplain Dale Denton officiating.
Spurlin Funeral Home is in charge of arrangements.
